Submitting clearances via DISS for visitors who hold an active DoD clearance:
Use SMO code 14213. The request must include the dates of visit, technical POC (i.e., the SNL host of the meeting), and purpose. If the visit involves RD, the visitor’s CNWDI approval must be reflected in DISS in the “ACCESS TAB”.
*Note that ‘interim’ DoD clearances are not accepted. The visit must be in an active state for us to be able to process. We need the individual on the visit, their clearance level and for RD CNWDI must be in the “ACCESS TAB” and showing active.
To process visits at the SNSI/SFRD level please ensure that the visitors visit request includes the following 10 items. Clearances must be sent via DISS (see below) -OR- on a company letterhead –OR- the DOE F 5631.20. Have visitors send clearances directly to SNL incoming visits to fax number 505.284.8812 OR email to incoming@sandia.gov.

- Social Security
- Citizenship
- Date of Birth
- Type of Clearance (Secret or Top Secret) AND Date of Clearance
- Dates of Visit
- Purpose of the Visit (no acronyms)
- Person to be Visted (must be a SNL employee, not security personnel)
- Place to be Visted (Sandia National Laboratories)
- Security Officer Signature and Security Title (the word “SECURITY” MUST be in the title)
To process visits at the SRD level have visitors send clearances directly to SNL incoming visits to fax number 505.284.8812 or email to incoming@sandia.gov. For DISS submittals we must have CNWDI in their “ACCESS TAB.”
Utilizing DOE F 2634.20 must include the following items…
- Name
- Social Security Number
- Citizenship
- Date of Birth
- Type of Clearance (Secret or Top Secret) AND Date of Clearance AND CNWDI access along with brief date.
- Dates of Visit
- Purpose of the Visit (no acronyms)
- Person to be Visited (must be a SNL employee, not security personnel)
- Place to be Visted (Sandia National Laboratories)
- Signature and title (title MUST be out of the DOD Directive 5210.2, Enclosure 4)
- All 3 boxes must be checked YES
- Specific Information to Which Access is Requested box needs to say SRD
